The original V7 sources had the kernel include files duplicated in two
places:  /usr/sys/h (in the kernel sources) and /usr/include/sys (with
the other include files).  A long time ago I decided to put them in
only one place, with the files in the other place being just nested
#includes of the authoritative copies.  I decided to put the "real"
copies in /usr/include sys.  It has become clear, on thought, that this
was a poor decision.  I have bitten the bullet and relocated them to
/usr/sys/h.  Similarly, the official copies of the ioctl include files
are now in /usr/sys/ioctl, with /usr/include/ioctl containing includes
for them.

In a (more or less) unrelated change, the SCCS archives for files in
/usr/include are now located in /usr/src/include.
