In 4.2bsd ROOTINO is defined in /usr/include/sys/fs.h as 2 typecast to ino_t.
I do 'grep thing /usr/include{,/sys}/*.h' a lot working closely with both
4.2 & Sys -V. You'll also want to include /usr/include/sys/types.h.

Many system things are in different include files under 4.2bsd. My comments
about that are not printable.
